About this Project

This innovation, ComRx, transforms referrals and health information data flow from CMS-funded health care sites to community-based organizations on Chicago’s South Side by: 1) generating automated, customized referrals through an interface between the EHR and a comprehensive community-based service provider (CBSP) database; and 2) delivering aggregate EHR data reports to CBSPs to inform and justify community health programming. The innovation will demonstrate improved patient and provider experience and retention; improved primary care and CBO utilization and impact; engagement of community youth to collect community referral data while learning about health, education, and advocacy, and gaining job readiness skills; and expansion of a trained community health workforce.
