About this Project

HC-LINC – Funded by the Robert Wood Johnson Foundation (#57398), Healthcare Community Partnerships to Prevent Diabetes (HC-LINC) was a cluster-randomized trial comparing two different practice-improvement approaches for implementing evidence-based screening, brief counseling, and referral services for adult primary care patients at high risk for type 2 diabetes.  The study involved collaboration with 10 primary care clinics and the leadership of a large metropolitan YMCA association and produced numerous pragmatic educational tools and reminder strategies for both patients and primary care providers.  Importantly, this research also led to the development of an integrated “Linkage Framework” to guide future work in the area of healthcare-community coordination for health promotion and disease prevention.
